In terms of a facelift, you might be a little early to have one.  It would really be beneficial to see a picture of you without smiling. Best treatment for tear trough: lasers, fillers, or fat injections?

The tear trough is a tough area to treat through eye lifts / blepharoplasty procedures. This area is located between the nose and the lower eyelid and is situated not above the bone as most people believe but above the soft tissue of the eye socket and eyelid. People have tried repositioning of the eye socket fat into that area through a blepharoplasty / eyelift procedure.  This can work but has been shown to lead to eye position abnormalities and problems with dry eyes, etc. Fat grafting can work in this area and may be the most natural way to rejuvenate this area.  It does have some issues related to survival of the fat.  But is the fat survives past a year it will likely stay in the area for many years, sometimes 10-15 years.   Fat grafting requires a lot of skill and knowledge of fat survival techniques.  Fillers can fill up the volume loss that has lead to the tear trough appearance and I’ve done that many times and have a lot of patients that get this done.  The key is being able to fill up the area without leading to bumps and ridges.  This requires some skill and with a surgeon’s experience it can be done to look very nice and satisy your clients.  Fillers can look not as natural as fat, it can lead to a translucent type of appearance and some dark appearance of the filler within the skin.  But, in general >95% or more patients are very, very happy that they had filler placed to treat the tear trough area. Lasers can create a layer of collagen underneath the eyelid skin to camouflage the dark appearance of the muscle underneath the skin.  It can also tighten the skin and reduce the unwanted wrinkles in the lower eyelid.   However, lasers will not replace the volume that is lossed with aging around the lower eyelid area.  This is where fillers and fat injections come into play.

How do you build up the cheek?

Building up the cheek can be done through multiple different means. This can be done with various fillers including restylane, juverderm, more radiesse, fat injections, or facial implants.  Either of these options have benefits and drawbacks.  The fillers are temporary and you will need reaugmentation in 6 months to a year.  Facial implants will last essentially forever but need to be done in the exact way and augment your cheek in the best way.  When you place cheek implants into the cheek area, sometimes the cheek can be fixed on the cheek bone with metal screws and if there is less than ideal augmentation more or less then you can tailor the augmentation through different means.  If you have too much augmentation, you can tailor the cheek implant with burrs if the cheek implant is solidly fixed with the screw.  If it is not big enough you can always place a bigger implant or  fix more silicone onto the existing implant with sutures. The key with silicone or other implants is to have the implant fixed so its stable, not moving and not collecting fluid between it and the bone.  These are things that can prevent infection and rejection. Silicone is probably the safest and most stable and non reactive implant that you can place.  Other implants create more inflammation and have a higher incidence of rejection and infection.

Fat injections are another way of building up the cheek.  The survival of fat is variable but if the fat survives after the first 6 months to a year the fat will likely survive for years.

Three days is way too early to tell what you are going to look like. Fat grafting, depending on the technique that you use, creates a lot of manipulation to the tissues. This is a consequence of having to layer the fat into multiple separate layers in order to help the fat survive. This manipulation can create a lot of inflammation after your procedure and this can lead to the swelling that you are experiencing right now.

Sometimes, this swelling and bruising can take several weeks to go away. I would definitely not do anything this soon. This could be a big mistake. If you do anything now you can have other effects that you weren’t intending to have. You should definitely bring these concerns to your doctor. Your doctor will help you through this. There are a lot of amazing doctors out there and your doctor, I’m sure, is one of them. Just make him aware that you are concerned.

When all of the swelling goes down, you will then be able to tell if you need anything else done. Don’t worry, there are always options to help your situation. You can always add more fat to make the fat that is causing your concerns blend into the surrounding area. You can always remove fat through an incision, or through micro liposuction. Steroid injections can also be an alternative to consider. One thing you could consider is oral steroids at this point in your recovery. This could help your swelling. But make sure that you know of the potential risks involved with steroid use.

Fat grafting or facial implants?

Some people are afraid of having implants and this an important thing to consider. I usually use implants to volumize the face when a person does not have a lot of fat to use, for example in women that are physically fit and have little body fat or if they don’t want to go through the harvesting of the fat that requires liposuction in another area of the body.

After implantation, you can then use the little fat that the person has to augment the implant. I usually do this in two phases with the implant first and then fat grafting at a later date. You also have to consider that fat and implants have different characteristics as well.

Implants require more incisions and usually are placed through the mouth. Fat transfer requires small holes instead of long incisions. Implants can also help in that fat transfering is needed less and cuts down on the need for more extensive fat harvesting.

The incision in front of the sideburn can avoid moving the sideburn back. This would be the only way to avoid moving the sideburn up.  So the decision is up to you what is more important, moving the sideburn up or a scar in front of your sideburn. Now for males, their beard hair can be adjusted with shaving to grow a new sideburn. This patient had a fat transfer procedure somewhere else and was concerned that she didn’t have enough fat near her ears.  She had fat placed in her cheeks and it made the lines around her mouth better though.

Sometimes, augmentation in one area may make another area appear under augmentated. The swelling should go down and reveal the final results in 6 months to a year. Your doctor will be able to tell if the volume is due to one area being larger and more swollen then the other and whether adding additional fat would be necessary.

You can always add more fat. Taking out too much fat is a little harder than adding more fat at a later date. The best way for me to tell is to see pictures and you in person.  The way you put fat in and where can mean a world of difference.

Fat transfer could be your answer. Many times, the cheek area and lower eyelids appear the way that they do secondary to a loss of volume in those areas. Fat transfer is the best thing to make the area look natural. It’s not magic, but when done right and with enough attention, it can seem like magic.

A midface lift, in my opinion, results in less than optimal improvement of the area. You can’t really take that much skin from a midface lift. Also, with a midface lift you can end up with eyelids that are pulled down like a hound dog. It also doesn’t look natural and you often get bunching in the lateral eyelid area. I think that a midface lift is only beneficial when you need to improve the lower eyelid when there is a problem with the lower eyelid function. Once you plump up your midface with fat, you can concentrate on improving the acne scars with different types of procedures including resurfacing and excision of your scars.

Losing volume in your face can lead to the nasolabial folds (or mouth folds, mouth wrinkles, mouth creases, nose lip wrinkles, nasolabial wrinkles) becoming more prominent.

  • A midface lift is not a favorite procedure of mine.  I think it shifts tissue upwards but it doesn’t address the nasolabial folds significantly.
