Dr Young’s ideas on Singer Charice and her quests to change her look through Plastic Surgery (Bellevue, Washington)

Charice is a wonderful singer as I have seen her perform on Oprah.  Recently, there has been information on her seeking the advice from plastic surgeons.  What has been talked about is her recent use of botox for her episode on Glee.  Botox is great for younger people to keep the wrinkles at bay.  Especially when they get nervous and start to create alot of wrinkles in their forehead, in between their eyes and crows feet.  In can give people the impression that they are more relaxed than what is really going on inside.  Many times when people get nervous they may lose their sense of how they are appearing.  They crunch their eyebrows and it makes them look mean.  They raise their eyebrows when they get nervous and you can tell they are nervous.  Botox comes all of those things down and you look better, more relaxed, and not as nervous.

She also is inquiring about make her face less round and what she can do to make her self look better.  I did an analysis on her based on my theory on facial aesthetics.  Although her face is tilted.  In general what the diagonal lines are showing you is that her cheeks and lower cheek areas are really dominate.  The middle oblique, and the vertial line is where the shadowing should be increasing markedly as it you go from lateral and inferiorly.  But  you see in the picture that her cheeks still are showing a lot of highlight in this area.  This is seen in the first figure.  The second are before and afters of my morphing to show you how she would look with some jaw and cheek reduction surgery and rhinoplasty.  I think she could look a lot better by narrowing her face and also making her nose a little smaller in the tip area.  She might also need some mesotherapy or microliposuction in her cheeks.

Botox or Dysport both work equally well in decreasing the muscle mass of the jaw muscle called the masseter.

Botox or Dysport both work equally well in decreasing the muscle mass of the jaw muscle called the masseter.  This procedure is very safe. I have never had any serious issues with this use. The amount of botox or dysport to use is dependent on the person because everybody has different responses and different volume of muscle. I usually tell this to people and then explain that I recommend starting at a lower dose like 15-25 units of botox for each side.  Dysport requires 2.5 to 3.0 times the units of botox for equivalent dosing.  Then I tell them that they can see if this adequate muscle relaxation in 2 weeks for botox and 1 week for dysport.  If there is still significant muscle movement of the masseter you can elect to do more.  One important point is that the muscle mass reduction lags behind the muscle movement.  It takes from 3-5 months to see the maximum volume changes in the masseter muscle and then it slowly increases the mass to normal by one year.  So, in short, there are some doctors that are seeing results from botox to the jaws for one year.  In my experience, the mass usually comes back earlier than this but the results are more longer lasting than what can be expected with other facial muscles.

Masseter and Jaw reduction to slim the face with Botox

Botox can be used to slim the posterior part of the face through injections that involve the masseter muscle.  This is done in the office.  We can use topicals and ice to make the injections more tolerable.  We basically inject the botox next to the bone and into the masseter / jaw muscles.  The botox weakens the muscle so that it eventually loses some of its bulk.  The botox takes 2 weeks for maximal effect but the volume reduction is different.  The volume reduces most rapidly in the first 1-2 months with the maximum at 3-4 months with slow reduction over the course of a year.  At twelve months, 50% of the volume that was decreased will be regained at this point. I usually tell people to wait at least a month to see how it looks before adding more botox.  The usually starting dose is 20 units.  But I find that it is most effective when 40 units are started for each side.  But the cost of botox can be high so usually people like to start at 20 units on each side.  Many plastic surgeons price botox less when more is put in.  When you put in 80 or more the price is usually around 11-12 dollars per unit.  Botox in the masseter can decrease the prominence of the jaw muscles so that the lower part of the face looks more slim and this can give the female face a more attractive appearance.  A larger lower third is relatively a musculine trait and less feminine.  Botox in the jaws and masseter muscle can also help some tension headaches that are due to muscle overactivity and temporal mandibular joint disorders.  People who have night time teeth grinding / clenching can have bad headaches.  Botox in the jaws is needed less frequently than the facial muscles for other cosmetic reasons.  Some people can get away with doing botox to the jaws on a yearly basis.
