What causes your neck muscle to loosen

Monday, July 6th, 2009

It is due to loss of facial volume, drooping of the neck muscle, laxity in the muscle layer and loss of the connections between your neck muscles and the deeper structures within the neck.  The platysma is the muscle that wraps your neck and this is attached to the facial muscles.  When you lose volume in the face from losing fat, tissues in your skin and the bones in your face getting smaller, this neck muscle descends.  Also you lose the connections of your neck muscle with the underlying deep neck structures with time as well.  I think that what also happens is that your posture is affected over the course of the years so that the cervical spine also becomes lax in such a way that the distance between your chin and cervical spine is shorter which contributes to the extra tissue between them.  Also as you contract more of your neck muscle, this muscle shortens with time.  The shortest distance between two points, your chin and lower neck, is a straight line.

Neck pain and swollen stitch in the neck area

Sunday, July 5th, 2009

This is a question that I answered for a patient who had something done from another doctor.

Pains and aches after any procedure including a neck lift is a natural process and sometimes the cost of being beautiful. It is almost impossible to not have these after any procedure. The question is how painful it is and if it is causing you to be in a state where you can’t function well on a daily basis. The stitch might need to be removed if it continues to cause you problems and can be done very easily.

The headaches are due to sutures pulling on your facial muscles and will get better. You might need some different medications to help you with your pain and transition.  Sometimes the pain is due to nerves being caught and is rarely indicated to have a procedure to alleviate this nerve impingment.  Muscle relaxants, anti inflammatories, Nerve relaxants can all help your situation and can be discussed with your doctor.  Don’t worry this will get better. Just be patient.  Your doctor will hold your hand, so to speak, through this whole process.

What to do about the neck bands a questions I answered for a patient who had things done by another doctor

Friday, July 3rd, 2009

You didn’t mention what was most bothersome but I’m assuming its the banding in your neck. Botox/Dysport can help improve this banding and will last 3-6 months. Another way of doing this is by doing a minimally invasive facelift concentrating in the neck area along with doing some work under the chin to release the bands. This approach is more invasive and another way of improving the way your neck looks is by grafting fat throughout your neck. You have to have enough fat to graft into the area. Usually the abdomen or stomach area is the best area to get this fat.

Looking for a non surgical way to make the double chin go away or improve it

Friday, July 3rd, 2009

There are many options such as losing weight. This can reduce the fatty deposits in your neck. However, depending on someone’s age, some of the double chin can be due to the muscles relaxing.

You have a neck muscle called the platysma that covers your neck from your chest to your face. This muscle during youth is tightly adherent to the underlying neck structures. When you age, the muscle eventually comes away from the neck’s deeper structures, contributing to the neck laxity and double chin appearance.

The main way to improve this is to do a neck lift along with a lower facelift. Liposuction is a very minimally invasive technique to slim down the neck and tighten the skin as well. But sometimes, the results you get with liposuction is limited, and more advanced techniques are needed to make your neck more shapely.

Augmentation of your chin can help create a greater disparity between your face, chin and neck region. Chin implants can help improve the angles of your chin with your neck. Other options that are less invasive include volumizing your lower face. This can have a tremendous impact on pulling the loose tissues up towards the area under your chin and also create more of a covering effect for the contents below your jawline.

The other way of improving this laxity is to use lasers to tighten the skin.  Active and Deep fx combined can have significant tightening effects.  This can be done without making incisions and with lower downtime compared with traditional co2 laser resurfacing procedures.

Lower Facelift or fat grafting and questions on the incision in front of the ear

Saturday, June 27th, 2009

The incision in front of the sideburn can avoid moving the sideburn back. This would be the only way to avoid moving the sideburn up.  So the decision is up to you what is more important, moving the sideburn up or a scar in front of your sideburn. Now for males, their beard hair can be adjusted with shaving to grow a new sideburn. But for females, when you move the sideburn up you will not have the sideburns back to completely normal after the facelift. Fat injections under the eyes and cheek area is a great way to make yourself look younger in a natural way. I would really like to look at a photograph to be able to tell you.

I refined an amazing volumizing procedure called “the YoungLift”.  This is an innovative technique that volumizes your face to bring out the younger and natural you without looking like you had something done.  This is not a facelift and requires no incisions.  Only pinpoint puncture sites are used and these heal imperceptibly.  The YoungLift can be done without general anesthesia and without drains, and large bandages.  You also have a lot less downtime and discomfort compared with traditional facelift procedures.  Dr Young employs the very best techniques from around the world into one volumizing procedure and he uses his internationally acclaimed understanding of facial beauty to create the youthful volume you once had.

Black eyes 2 months after a facelift by another surgeon

Friday, June 26th, 2009

Sometimes, black eyes are due to many factors that can thin your blood or make you bleed more during surgery and afterwards. Genetics, medications, technique of the surgery, etc can all play a part.

When you ooze more during and after your procedure, you will tend to develop more bruising. Two months is rather a long time to resolve this, and it may be due to your circulation and general health. Also, with a facelift, the lymphatics that drain your eyes and the pathways from eyes to your general lymph system and circulation are sometimes impeded due to the area of the facelift. Your eyes drain towards the angle of your jaw, below your ear. This is the exact area that is manipulated during the facelift and can impede the drainage of your black eyes.

Exercise and sometimes massage and warmth can help. Black eyes can also be due to iron deposits from blood where your bruises where located. This is harder to treat but can be the same treatment as with persistent bruising, i.e. exercise etc. Sometimes, people also attribute prominent veins to the appearance of black eyes.  If these are veins or blood vessels, time will improve them. Sometimes lasers can help decrease the appearance of these vessels. The only way you can find out is by having someone look closely at it.

This is a question that I answered for a patient that had lumpiness, and numbness after a procedure by another doctor

Friday, June 26th, 2009

This is a question I answered regarding a procedure done by another physician.

Lumpiness, swelling, and numbness after a facelift / mini lift / s lift / weekend face lift can be a normal thing. Lumpiness is usually due to a number of factors of which include; fluid collection, inflammation, sutures, and cysts.

Most of the time, the lumpiness will go away with time. At times, your surgeon may consider steroid injections to help the lumpiness. Although, time is a better alternative.  Steroids injections can cause their own problems like skin thinning that is hard to predict along with vessel formations and other side effects. Massage is another thing that can help with the lumps.

Heat is something that I would be really cautious of employing. Numbness is a natural process. When you elevate the skin for the facelift, it will interrupt the nerves reaching the skin and this is just part of the process. It takes nerves one day to grow 1mm in the best of conditions.

Many times it will take up to a year for the nerves to fully come back and sometimes the numbness does not fully recover. Your doctor should help you with this and hold your hand so to speak through the whole process.
