We are going to discuss a little bit about the before and after photographs. It is important to take a look at these photographs to get a sense on what the surgeon can do. Rhinoplasty is the most difficult procedure that plastic surgeons do. The delicate cartilages in the nose and the 3 dimensional aspects of the nose create many challenges. One thing I see out there in the internet is the presentation of before and afters showing only the side view. It is really important to realize that this presentation exemplifies mostly a 2 dimensional view of the nose. It can reveal how the bridge is related to the tip of the nose and whether any hump was reduced. But the 3 dimensional aspect is much more difficult to achieve a desired result and this is much more revealed on the pictures that show the front of the person’s face. I would always ask to see the front view of the person’s nose. This is the view that you can determine if the tip or bridge is small or shaped enough. This is not revealed on the side profile view. If you see a website that predominately shows the profile / side view, I would be more concerned that they are not showing you everything that you need to know. Ask for the front view to get a better idea of what the surgeon can accomplish.

I just want to take the time to thank all the wonderful people that I have encountered since starting our practice. I meet wonderful people everyday in the clients that I meet. A patient today for example was talking to me about The Seven Spiritual Laws of Success by Deepak Chopra. I was so excited to talk about this wonderful book about finding your potential, the law of giving, and the law of Karma. I totally believe the laws of this book. As I tweeted earlier, it is now like I was blind and now I see. Since starting our practice, we have grown and become very popular and very busy, even in a down market / economy. I believe that some of this success is not just from my hard work but it is also from our giving on a daily basis. Our clients are our number priority and this attitude and directive in us has served us well. This was our goal even before our delving into the seven spiritual laws. But now with his teaching we will even approach this with more vigor. Since we have grown, we have also attracted a lot of attention from our competitors. Some of our main competitors have seeked out less than positive approaches towards our popularity. Some of these challenges that our competitors have brought to us have been very unethical and even devious in many eyes. I would like our patients, staff and all the people affliated with us to know that we are going to be strong and proud through this difficult time. In fact, instead of fighting fire with fire, we will devote our selves to more giving, more positive approaches to life even more. We refuse to fight back in their unethical ways. We desire to give more love to world, more positive feelings in the world. We wish to spread the beautiful seeds of life, to spread as in nature the seeds that lead to the growth of the most powerful trees that we see around us. We will practice the Law of Giving as written by Deepak Chopra. As Gandhi once said “Be the change you want to see in the world.” Life is Beautiful.

We are so happy that we have made such improvement in people’s lives through our scar revision techniques. We wanted to talk about our scar revision testimonial page. We have people who travel from around the world to have us work on their scars. This testimonial page for scar treatment involves two people who traveled from the Bahamas and Minnesota. We are very happy to present these testimonials and hope to add more on to this page. This is something that we just started doing to help people understand the things we do. For scars we have many approaches to improving your scars and we will tailor an approach just for you. You can look through our Scar Treatment Page to understand more of the techniques that we do. Our philosophy is that we try to get the scar to look more like the surrounding normal skin and the closer it gets we then decide to do some polishing with lasers, dermasanding, chemical peels. To get the scars ready for that will entail excisions, deep chemical peeling, geometric line closures, subcisions, fat grafting, etc. Based on your scar we will figure out the best approach.

In my experience I’ve had some patients that have some pretty interesting requests when it comes to where they would like to place their Dysport or filler. So I’ve decided to put together a little list of where ideal places would be for Radiesse (filler), Restylane (filler), and Dysport (wrinkle reducer).

However, let me first give you an overview of these 3 products and how they differ. Dysport which may commonly be compared to Botox is a substance used in most cases to restrict the movement of face muscles , helping to reduce the creation of wrinkles. For instance the forehead, we would inject in this area to relax the muscle and then the lack of movement causes the wrinkles to smooth. Moving on to Restylane, this is a product used to help volume loss in the face and also helps to fill in lines. Radiesse although similar to Restylane is a product that also helps with volume loss and fine lines however, it tends to last much longer then Restylane.

So here is a list of possible areas to inject. Let’s start with Dysport!

1. Crow’s feet (around the eyes)

2. Glabellar (between the eyebrows)

3. Forehead

Restylane:

1. Eyelid troughs

2. Malar Crests

3. Lips

Radiesse

1. Nasolabial folds

2. Marienette Lines

3. Cheeks

So here are just a few of the uses of these products just to give you an idea of where to start.

Although some of these spots may not be considered “On Lable” uses, they are still used frequently in todays practice.

Hope this helps!

Lip augmentation can be great, especially if you have realistic expectations. Restylane in the lips can be a good choice to pick for temporary and sometimes longer lasting effects. However, I feel that the lips can be tricky, especially for the patient. Restylane is composed of hyaluronic acid – this substance for some patients can help build new collagen in the lips. This explains why some patients have longer lasting results than others.

When injecting patients I feel the patients can sometimes become distracted by the seemingly “instant” effects. The lips are highly sensitive to the injections, not just because of the product but from the needle stick itself. It can at times look as though the patient is getting a “fat lip”. In my opinion I feel that before getting lip injections the patient should be aware that there will be swelling at first. However, this is temporary.

One more point about Restylane. Lip injections can be helpful to get rid of lines and create a nice pout. However, I feel that the lip can only hold so much before it starts to look, “Hollywood” or “fake”. Then there is always the option of lip implants, However that is entirely new ball game

We recently had a patient request releasing her crease from a previous Asian blepharoplasty. This can be done by an experienced facial plastic surgeon. It entails releasing the crease by releasing the scar tissue and the attachments of the skin above and below the crease from the levator muscle. Then you can have to sewn and position tissue between the skin and the levator to prevent further future adhesions. A thorough discussion with the patient is essential in doing this and knowing what exactly they want. If the patient wants the crease higher, then the plan is to find out how high the patient wants and the new incision is designed usually in a higher position. Sometimes it will entail release of the old creaase or just tightening the crease in the higher position. The key is being able to identify the anatomy without causing serious damage. To lower the crease the situation can be more difficult. Experience can play a big part. I have a mathematical way of approaching this situation. It helps me figure out where I will put the future crease and anticipate how low to make the new crease, etc. If you have questions you can email us at our facial plastic surgery contact page.

We just had a patient from Japan. I was really impressed that this person came all the way over here and picked Aesthetic Facial Plastic Surgery, Dr. Young and us as a team. We performed the YoungVitalizer formerly known as the YoungVolumizer. I thought that was really amazing they traveled so far to come here. I think that is awesome that we can take tissue from somewhere else and that you can put it into your face and make it look smaller again. I thought that the face got bigger as we got older. Our patient from Australia has never been to Seattle / Bellevue before and she was really excited to be here. I thought it was interesting to hear an Australian accent in an Asian person, it was really appealing.

There are differences in how you do a double eyelid surgery for the asian and even non asian eyelid. Asian double eyelid surgery is most often done with the dermal levator fixation, where the skin is connected to the levator muscle. You can see the relevant anatomy below. The sutures are usually removed 6-7 days later. This is done almost 95% of the time. The double eyelid in this situation is dependent on the formation of scar to form the crease. Although effective most of the time in creating the crease, the crease is felt by some more experienced surgeons to be less natural then some other approaches. We advocate the double eyelid surgery using the orbicularis fixation technique where the orbicularis muscle is connected to the levator and the suture is left in there permanently to hold the crease. This leads to longer lasting results and also a more natural working crease. The crease tends to relax when the eye is closed and work when the eye opens, just like it does in a naturally occuring crease. The dermal levator fixation however tends to have a persistent crease even with the eyes closed. Some patients don’t like that feeling and feel that it is an unnatural result.

A medial epicanthoplasty is a technique of improving the appearance of the middle part of the eye. In asians and other ethnicities, there sometimes is present a fold over the fleshy part of the eye near the middle. This procedure allows the eye to become more open in the lateral direction and the results can be a markedly much more open eye. Patients who have this done are usually very happy with this procedure. We advocate a special technique that leads to much less scarring as compared to more traditional and older techniques. You can go this link to read more: Asian Eyelid Medial Epicanthoplasty.

The smile releases endorphins that have been shown to have an effect on your health in a positive way. Endorphins have been shown to help your white blood cells work better which are known to fight off cancer, bacterial infections, viral infections, etc. Not only does smiling improve your health, mood and success, but it is contagious too!

This is our blog on our procedure to place dysport into the muscles of the Jaws to slim down the face. This is a really common procedure that we perform on our Asian Female Clientele and for patients with tmj issues. Below is a picture of a result that you can get with dysport in the jaw muscles. Placing the dysport in the right place is very important to get a good result. If you don’t place the dyport in the muscle itself you will not get the benefits of this treatment. The first important thing about getting dysport in the jaw muscle is that the muscle will get fully weak at about 1-2 weeks. It is faster with dysport than botox, about a week. Although the muscle is the weakest 1-2 weeks after administration, the decrease in size of the muscle can take up to 3-5 months to be fully realized. We will help you in understanding this more when you come in.
