Should I get a browlift or eyelift or both?

A browlift raises the eyebrows upward to improve droopy brows, while a blepharoplasty takes away extra eyelid skin.  When people age, the eyebrows tend to descend and create more tissue in the upper eyelid area.  Lifting the eyebrows can move this accumulated tissue away from the upper eyelid.  Sometimes removing tissue from the upper eyelid can enhance the total appearance of the eyebrow and upper eyelids.  How do you know what to do? Well your surgeon will help you with that.  But essentially your eyebrow should be above your eyelashes by about the width of the colored portion of your eye (or iris).  To have a more awake look, this can be a little more.  When it becomes more than 1 1/2 of an iris width a surprised look begins to be sensed by the viewer.  After determining the ideal height of the eyebrow, any extra eyelid skin can be removed to enhance the whole upper eyelid, eyebrow region.   In terms of numbers, the eyebrow should be at least 1.2cm away from the eyelashes and not more than 1.8cm away to avoid the surprised look.  If the eyebrow is already 1.2 to 1.8cm away from the eyelashes, an upper eyelift could be the only thing you need.  Even better, as you age, you really lose volume throughout your whole face.  Volumizing is a great alternative to the incisions that are necessary with browlifts and eyelifts.  Volumizing can be used to elevate the eyebrows, fill in the upper eyelids in a natural way with incisions, significant discomfort that is associated with older, traditional procedures like the eyebrow lift and eyelift / blepharoplasty.  This is where the YoungVolumizer can help.  The YoungVolumizer is a procedure I developed to volumize your face in a natural way with your own tissues and without incisions, and the discomfort of traditional, older procedures.

How many syringes do people use for certain areas on average when it comes to fillers in the face?

There is no real standard for fillers (Aesthetic Facial Plastic Surgery, Dr Young Bellevue) in certain areas but a general idea is out there.  One thing to realize is that everybody is different.  But in general, for example, in the lips I usually start out with one syringe of restylane or juvederm.  For the nasolabial folds, I usually use at least two syringes of either one.  For the cheeks you can 2-4 syringes.  For the under eye area along the inferior orbital rim, I use one on each side to improve the hollows. For the upper eyes and the hollowing there I use 1-2 total.  For the marionette lines under the lateral part of the lips I will use 1-2 syringes.  The syringes range from 500-700 per syringe on average.  There are some reductions the more you do for most offices. Instead of fillers, options that I think are great include fat injections, or the YoungVolumizer which I developed.  With these options you can globally volumize the face and given a general rejuvenation.

I had 2 facelifts and my surgeon is suggesting fat grafting to my face for facial rejuvenation by Dr. Philip Young Bellevue | Seattle: Fat is a great option after multiple facelifts (Dr Phil Young Seattle / Bellevue Washington) to volumize the face.  Aging can be seen as a process of changing from a grape to a raisin for lack of a better descriptive scenario.  Facelifts essentially make the raisin into a smaller raisin. When in fact to look younger a lot of times looking like a fuller younger naturally looking grape is the better option.  This is where volumizing plays an important role.  Fat injections volumize the face to return the raisin to the grape that it used to be.   Given that you have had 2 facelifts, you likely have a reduced skin volume and hence you will be smaller than the grape you once were, unless you gained alot of weight during the years.  Volumizing would be excellent for you likely.  But it all depends on what you look like and this is a general comment.  At the age of 74, you can still get benefits from facial fat grafting.  I have done a lot of patients over this age. It all depends on your health.  Of course you need enough fat to do the procedure and your surgeon can tell you about that or assess this situation. Can lasers improve the turkey neck?

Lasers can improve the Turkey Neck but to a minimal degree.  This is a good option for people that are not ready for surgery.  The laser can tighten the skin, improve the wrinkles and decrease the aging that occurs with sun damage.  One thing to consider that it is better to be safe than sorry in the neck.  The neck skin is thinner and has more potential for scarring and other healing issues.  So being conservative is vital in the neck area.  But to definitely treat the neck a traditional neck lift is the proven way to rejuvenate the neck.  Another option is fat injections and the YoungVolumizer for the neck. After all, a big part of aging in the neck area is a loss of volume under the skin. Here is a live demonstration of my use of a co2 laser and laser resurfacing.

What fills in the hollow under the eyes, do implants work there?

Eye / Inferior orbital rim Implants can fill in the hollows under the eyes. Volume loss in the cheek and under the eye along the inferior orbital rim can lead to the hollowing under the eye that makes the bags under the eyes stand out.  There are many ways to fill in that hollow. You can use fillers, fat injections, the YoungVolumizer, Implant (Dr Young Seattle Washington), and you can sometimes reposition the fat over the rim.  Taking out the lower eyelid fat reduces the baggy appearance but doesn’t correct the original reason why there is a hollow there in the first place which is due to volume loss. Do non surgical cosmetic treatments really work?

Options for non surgical cosmetic treatments include: lasers, fillers, sun avoidance, topical skin treatments, etc.  All of these modalities have proven to show improvements in a person’s aesthetics.  Lasers can non surgically remove sun spots, tighten your skin, reduce vascularity and red spots, improve skin texture.  Fillers, can fill volume loss in your face which is actually the culprit of most of the aging that we experience as the years go by.  Sun avoidance decreases the aging of your skin that occurs when your skin is exposed to sun.  The UVA and UVB light can cause DNA mutations in your skin cells that cause aging and also can lead to skin cancer.  Skin treatments which can include microdermabrasion, topical creams, facials, chemical peels that can be done in office and at home can all lead to skin rejuvenation.  Skin treatments can tighten your skin to a degree, decrease unwanted pigmentation, speed skin turnover to rejuvenate your skin on a daily basis and prepare you for significant treatments.

Another in between option is the YoungVitalizer (Dr Young who is in Bellevue, Washington discovered the YoungVitalizer).  This is a procedure that I invented that entails volumizing your face with your own tissue. As we age we change from a grape to a raisin and using your own tissues to volumize your face can make your face look more like a grape instead of a raisin with the wrinkles and loss of the curves.  Most traditional procedures change the aged raisin to a smaller raisin.  You end up, often times, looking like someone else instead of the youthful grape you used to look like when you were younger.

What is the muscle that causes crows feet?

The muscles that cause crows feet are the orbicularis oculi and the smiling muscles: levator labii superioris, zygomaticus major and minor.  Here is a picture of these facial muscles.  When we try to improve crows feet with botox, these are some of the muscles that we try to weaken to decrease the formation of wrinkles.  Botox is injected in the sides of your eyes in the skin to weaken these muscles.  By doing this, we make the muscles weaker so they are less likely to form these dynamic wrinkles.  Dynamic wrinkles are the wrinkles that show up when you move the muscles of your face.  They are more pronounced when you move your facial muscles.  Static wrinkles are the wrinkles that are so etched in that they are present when you are not moving your facial muscles of expression.  Dynamic wrinkles are mostly effected by the actions of botox.  Over time, with continued use of botox, the static wrinkles can become more improved.  This is done because with botox, eventually allows the skin to remodel and reduce the static wrinkles.  This action of improving the static wrinkles however takes a long time and requires regular and consistent use of botox.

What are the options for Crows feet?

Crows feet are due to the eye muscles that surround the eye. These muscles are meant to close the eye, to blink, to protect the eye, etc.  When doing these movements, they can affect the skin around the sides of the eyes.  What results with this movement are the wrinkles that are commonly called the crowsfeet.  Botox is usually the way that people get some relief for them.  It usually takes about 8 – 12 units of botox on each side and the costs are around 10-16 dollars per unit of botox.  Other options include laser resurfacing, fillers, or fat injections for these wrinkles.

Fat injections and fillers function to do the same thing. When we age we lose volume in the skin and in the whole face in general.  What this does is bring the skin closer to the muscles of our face.  When we were young, there was a greater distance from the muscles of facial expression and the skin.  This distance was occupied by our skins fat and connective tissue.  When you smiled when you were young, the muscles tended to glide under your skin.  The fat in the skin above the muscles of facial expression allowed the skin to slide.  This exact phenomenon occurs in multiple areas of your body.  It is essential for muscle movement in fact.   As we age, the muscles of facial expression become closer to the skin, when we progressively lose the fat and connective tissue within and under the skin.  Hence, progressively when we smile are muscles of facial expression begin to exert more effects on the skin and these are manifested through wrinkling when we smile.  Through fat injections, fillers, or the YoungVolumizer, this layer of fat between the skin and muscles is increased and the muscles of facial expression have less of a wrinkling effect on the skin.

Resurfacing through chemical peels, lasers, or dermasanding / dermabrasion essentially takes away the top layers of the skin to allow skin cells deep within our hair follicles, glands etc to “resurface” the skin.  In the process of the procedure, we remove wrinkles, create a new layer of collagen underneath the skin, remove unwanted pigmenation and DNA damage, remove precancerous or cancerous cells, etc.  The skin then becomes renewed with healthier cells, you have less sun spots, wrinkles and the tone and texture of your skin is rejuvenated.

Facelift for Acne Scarring and other options by Dr. Philip Young of Bellevue | Seattle:

Question: Is a facelift the best option for acne scarring? What other options do I have?

Answer: A Facelift is not the first option but I have noticed improvement in Acne Scarring from this procedure.  I personally have many patients that have noticed a significant improvement on their acne scarring  from a facelift procedure.  Is the facelift a procedure that is usually recommended to a person to improve their acne scars? The answer is “no”.  But I usually present all options to the patient.  Among these options include laser resurfacing, chemical peels, dermabrasion, dermasanding, fillers, fat injections, and excisional techniques.  It depends on how your scars look like.  If you have alot of deep ice pick scars you may need some excisions to completely remove them.  Of course if they are everywhere you might have to pick and choose which ones to excise or cut out.  This excisional approach is usually followed by some resurfacing.  I usually prefer to excise and then resurface the areas at the earliest 6-8 weeks.  One important point to remember is that the goal is improvement.  Your skin is probably not going to ever look like your skin when you were a baby of course. But we can make it better!  Fat injections and fillers can be used to break up the scarring that sometimes holding the skin down to the deeper layers.  This often causes the scars to be held down.  Fat injections is the best way to soften up the adhesions of scarring to allow the skin to come away from the deeper scarring. Question: What is the cost to improve the double chin and droopy cheek appearance.

Answer: The pricing on this varies depending on location and who you are seeing.  Some doctors have a better reputation and will charge a lot more for their services.  This reputation is usually based on results and word of mouth.  The pricing that is mentioned above will usually not include other fees assessed for anesthesia who have their own fees for these procedures and they usually are based on the time it takes to do the procedures.

The procedures that will help each area will include the necklift (Dr Philip Young of Bellevue, WA) and facelift for the double chin area. Droopy cheeks can be addressed by midface lifts, and deep plane facelift Bellevue area.  My personal opinion is that droopy cheeks are best augmented by volume and I prefer fat injections and the YoungVitalizer for this particular issue.
