New Transgender Ad Discusses, Demonstrates the anxiety that the transgender population faces when going to the Bathroom

There have been numerous recent stories taking on this new topic. This ad comes at the heels of new anti-transgender bathroom bills that are popping up all over the country. For example the North Carolina Bill:

North Carolina’s General Assembly voted on Wednesday to block cities and counties from passing protections against LGBT discrimination in a wide-ranging bill that could have enormous implications for the state.

This bill that passed a special session would set a statewide anti-discrimination policy, banning employers and businesses from discriminating against employees or customers based on their race, color, country of origin, religion, age or “biological sex.” The problem is that the bill offers no protections for the LGBTQ community and prevents local governments from passing any nondiscrimination policy that goes beyond the statewide standard. Meaning it, paradoxically, can promote or make legal discrimination against the LGBTQ community. This would leave areas governing wages, benefits, employee protections, and leave specifics unsafe for possible discrimination for the LGBTQ population.  This would also prevent people with gender identity disorder from using the bathroom of the sex that they identify with. Many people feel that this is the same as what used to be discrimination against minorites just a few decades ago.

What are some surgeries that you can do to make your face change into a different sex. One of the main procedures that Dr. Young believes will make a big impact is a Rhinoplasty. This is an easy way to change some of the basic dimensions of the face without major surgery. For feminization, we will want to make your nose smaller and more refined. To masculinize it we want to make it bigger, wider and taller with more of a bridge. Hairline lowering with brow bone reduction is something that can markedly feminize your face.  Along with that a brow lift will markedly soften the appearance of your eyes.  Doing the opposite will masculinize your face. This is a question that I answered for a patient that had a surgery done a while ago by another surgeon:

5 days is too early to judge your results after genioplasty. Right now you are going to have a tremendous amount of swelling.  You have to wait at least a week and up to a month for it to really look good.  Generally, I tell people that you will have 60% of your healing at 6 weeks, 80% at 6 months and the rest occurs up to 2 years later.  So be patient. It will get better and most of the time things can be changed back to the way they were or altered a bit.  I personally prefer chin augmentation to genioplasty.  Chin augmentation is generally, through many studies, associated with less complications than genioplasty.  These complications can include, nerve damage, a step off deformity, bone loss, problems with the implants or titanium plates and screws used for the fixation of the chin implant or the genioplasty segment that is advanced.

Browlifting with Brow Bone Reduction, Hairline Lowering, and Removal of Frown Muscles can Markedly Improve Your Appearance

There are many reasons for browlifting (Dr Young specializes in Browlifts and Facial Plastic Surgery) and working with the upper third of the face.  One of the main reasons is to lift the brows when they are drooping.   In this case, raising the eyebrows can open up the eyelids and reduce the upper lid skin laxity.  This can be done through open approaches, through the eyelid and through minimally invasive means through small incisions behind the hairline.  What can also be done is removing the muscles between the eyes that causes the frowning.  This can reduce your needs for botox markedly to not having to need botox at all.  Some people also have significant brow prominence that can be reduced by burring the bone down.  this can be done through an eyelid incision or through an open approach which requires an incision from ear to ear.  Although the incision from ear to ear sounds daunting, the incision actually heals really well when you close it accurately. Also through an open approach you are more likely to get longer lasting results.   Reducing the brow bone can reduce the stern and masculine look that some women have.  Hairline lowering is something that can also be done for people with high hairlines. This is done through an incision right at the hairline.  This combination of procedures, browlift, brow bone reduction, hairline lowering, and brow bone muscle reduction is much more commonly done for certain patients that want to have more of a feminine appearance.  This typically is more common among my transgender patient.  Below is a picture of a just such a patient.  Although we have done this for many women.  Most of the women we have are more conservative in showing their pictures on our website.  Notice in the pictures that his brow bone is less prominent and his eyebrows are elevated to his desired height that we discussed prior to the procedure.  Other procedures that he has were a chin reduction, rhinoplasty, and permalip implants.

Transgender Surgery Involves Multiple Steps

There are multiple procedural steps for someone interested in Transgender Surgery (Dr Young specializes in Transgender Facial Surgery in Bellevue, Washington).  The order by which you undergo these procedures can be individulized based on the particular person’s needs and anatomy.  1. Rhinoplasty, 2. Lip Augmentation (including Lip Lifts, Lip Plumping, V-Y Advancements, Lip Implants / Surgisil), 3. Chin Reduction Procedure, 4. Browlift, Brow Bone Reduction, Hairline Lowering, 5. Cheek Implants, 6. Fat injections to soften the facial structures, 7. Tracheal Shave / Thyroid Cartilage Reduction, 8. Angle of the Mandible Reduction or Botox Mandible Reduction, 9. Other Adjunctive Procedures including Blepharoplasty, Hair Reduction, etc.  The patient below had a conservative rhinoplasty, Permalip Implants, Lip VY Advancements, Chin Reduction, Browlift with Brow Bone Reduction, Hairline Lowering procedures.  We are planning on Cheek implants to heighten the highlights of his cheeks, fat injections to soften all of his features, tracheal shave in the future.  Notice his higher eyebrows.  Now this can be interpreted as being high for some people tastes.  This was something that we had discussed with the patient and he likes the brows little more elevated.  The ideal should be that the distance from the bottom of the eyebrow to the eyelid margin should be about one to one and 1/2 iris widths.  Also notice the difference in his chin and the decreased prominence of his chin.   He also had permalip lip implants to enhance his lip’s size. The VY advancements in his lips has created more of an eversion to his lips so that it shows more red in his pout.  The other procedure that we did for this patient is that we also resected his brow muscles that are between his eyes.  These muscles pull the eyebrows closer together and give a person a stern, mean look.  These muscles also created the veritical and horizontal lines in between the eyes.  Resecting these muscles decreases a persons ability to do these maneuvers and decreases the wrinkles there.   Also it allows the brows to become higher naturally because the brow depressor muscles are weakened.

Who are the best physicians to lower the hairline, elevate the eyebrows, and reduce the brow bone?

Facial plastic surgeons are excellent physicians to lower your hairline and reduce your brow bone prominence which is done through a traditional browlift (Dr Young specializes in Facial Plastic Surgery near Seattle, WA) approach. This is usually done through an incision just in the front of your hairline.  It requires some release of the hair behind it and advancing the hair line forward by tacking the hairline to the bone anteriorly and in a more forward position.   A brow lift can then be done in conjunction with removing of some of the muscles that make you frown.  And during this time, you can bur the bone down to reduce the prominence.  I often do this for my transgender population and for my female patients that have a more prominent brow.  Reducing the brow bone can also be done through an eyelid incision as well as removing the muscles that cause frowning.  Another option for lowering the hairline can be through hair transplantation to the front of the hairline by taking hair from the back of the scalp.

High Hairline and options for Improving this Masculine Look.

If you have a high forehead there are options for you.  Lowering your hairline is an option that can be done.  Hair transplants are one way of doing this.  This approach is highly technique dependent and requires a lot of time to do this.  Another way of doing this is to lower the hairline through a procedure that requires an incision within the front of the hairline that is a variation of a Brow Lift.  Then what is needed in elevation of the hair posteriorly and a little bit anteriorly.  Release of the scalp’s tension is then sometimes needed to allow the hairline to come forward.  Anchoring the hairline in a lower position is then done by fixing the hairline to the bone of the forehead. A browlift can also be done in concurrence if needed.  Further elevation done to the orbital bones is required for a browlift and after the release of the eyebrows you can then elevate the eyebrows to the desired position.  A look at myFacial Beautycan help one to figure out how high the forehead should be.
