Chin Implant Revision for misplacement and for aesthetic reasons (when the implant is too large)

There are many times when I get people that come to my office wondering about changing the appearance of a chin implant that they had before.  I have this case of a woman who had a chin implant by another surgeon and she felt that the chin implant was riding too high. She felt that the chin implant was also too large and it made her look a little masculine.  When it comes to revisions, you should not ever just take an implant out unless it is a really small implant.  What happens is that the soft tissue can ball up and contract and create an unfavorable appearance.  What is typically the better thing to do is to replace the large implant with a smaller one.  If the implant is already smaller, then you could make a case to not put any implant in and this is an option only for a small implant.  Generally small implants are the ones that people have an issue with.  Also when you put an implant in, they implant should alwaws ride on the inferior border of the mandible where the bone is hard and less likely to undergo resorption.  I prefer medpor implants for many reasons.  They can get incorporated and vascularized to act like your own tissue much more so than silicone implants.  Silicone implants tend to have a capsule form around it and will not get incorporated. Hence, when you hit your silicone implant there is more likely the chance for complications years after the procedure as opposed to medpor where once it gets incoporated past 12 weeks, it will be more resistant to infection and rejection. What is a good option for something in place of Sculptra? What are the alternatives?

Fat injections are the best option as an alternative to Sculptra injections. I even consider fat injections first before Facial Fillers. Nothing is better than using your own tissues for plastic surgery purposes.  You have essentially no risk for reactions, no risk for cancer, no toxicity from your own tissues. Sculptra is made up of suture material. It is a foreign substance that is essentially broken down into carbon dioxide and glucose and ultimately carbon dioxide and water.  Other ways of filling is through silicone implants. The problem areas for implants and Sculptra is the periorbital area.  Implants are also hard to use for the buccal area where there is no bone for the implant to rest on.  It is here that Sculptra can be used and Fat injections.

Can you Fat Graft / Transfer Around a Facial Implant? Can you Fat Graft / Transfer / Inject into the area if you remove the Implant?

You can graft around an implant but it depends on the blood supply around the implant.  Typically, around the implant a layer of scar tissue usually forms. Scar tissue is notoriously known for its low blood supply.  Bringing fat next to the implant has the potential to live if done right.  Over time with the survival of fat around the implant, additional fat grafting (or Fat transfer / Fat Injections / Fat Transplants / Fat augmentation / Fat enhancements) can be done there to further increase the volume around the implant. If you remove the implant, the success of fat grafting can be attained but within the scar tissue, the fat has a more difficult time to survive.  This may mean that you will need to do more touch ups to the area.  Fat grafting can lead to long term results.  It is all about the technique.  In my practice, fat does survive and sometimes near 90% of the fat can survive.  This is true, so much so, that I need to be careful not to put too much fat in to certain areas.  It is easier to put more fat into the area than to take it out.     Studies looking into the percentage of body fat are not prevalent in the literature. So this hasn’t been determined accurately. Because you work out regularly and have low body fat, you might not be the best candidate for a lot of Fat Transfer(fat injections, fat transfer, fat filler, fat injectible). I would reserve fat grafting for areas that are least likely to tolerate more permanent fillers or implants.  These areas would be around the eyes and in the cheek hollow area.  The alternative for the areas other than that are sculptra, silicone implants for a more semi permanent or permanent options.  Temporary fillers can be used everywhere essentially.  Temple hollows, forehead volume loss, jaw shrinkage and sunken cheeks can be augmented with silicone implants as an option in the very thin or fit patient.  Fat can then be used in the areas that cannot tolerate these implants.

    Can radiesse / radiance be used in the Jawline and lateral portion of the Jaw area?

    Radiesse can work but you may need a bit more in this area. Radiesse is made of calcium hydroxyapatite, the same constituent as your own bone.  This makes it really compatible with our bodies. For Mandibular augmentation, you will likely need a siginificant more amount of augmentation here than you would along the mouth lines.  Mandibular silicone implants are made up of approximately 10-15cc each side. So you will need quite alot of radiesse to show the same amount of change.  This is important to know before doing radiesse injections in this area.  If you just want to get a little taste of what this will look like then radiesse would be a good trial.  Silicone implants are one of the better ways of really augmenting the lateral jaw bone.

    When can you time Chin Implant Surgery with Braces?

    Chin implant surgery (or chin augmentation / chin enhancement / chin reduction / jaw implant surgery / chin silicone implant surgery / chin shaping / chin reshaping) occurs at the bottom of your chin and jaw area which are not in the same vicinity as your teeth and braces.  The only concern that I would have is regarding the hygiene in your oral cavity.  With braces, you are more likely to have residue in your braces and around your teeth and the bacteria load is likely to be higher.  Because of this I would rather do the approach through the bottom of your chin where the incision can hide really well.  Otherwise, I think you can do it with having braces.  Of course, I would wait until you are adjusted to any period where tightening of the braces occur for your comfort.

    Can fat injections be permanent? What are some options.

    Fat injections (fat transfer / fat grafting / fat augmentation) can be “permanent”. One thing to realize is that fat injections and anything in life aren’t necessarily permanent and forever.  Fat injection results can last many years if done right.  Just like the fat that you had when you were younger, they eventually went away after a while as you aged.  This same process will eventually happen to the fat that you have injected.  Over many years, the fat will get metabolized as you age but the results may last for 10 years or more.  Another option of volumizing is through facial implants.  Silicone is the most common implant that people place within the face and these implants can last essentially forever.  Facial implants (silicone implants / cheek implants / jaw implants / facial implantation / chin implants) are more of a permanent option but the drawbacks are having a foreign material placed within your face and it does require more incisions and likely recovery time than fat injections.  If you can handle the idea of putting something foreign into your face this is a great option.  Silicone implants have been placed into thousands of patients around the world and has an excellent track record for safety.  You can see more on fat injections on my website and youtube account.

    How is a chin implant surgery done

    Chin Implant Reshaping(or chin augmentation / chin plastic surgery / chin job / chin cosmetic surgery / chin enhancement surgery) are usually done through two approaches.  You can make an incision under your chin or in your mouth to place the implant.  The most common way to do it is through an incision under the chin.  This usually leaves a barely visible scar.  The incision is around 2-3 cm in length and the implant is place on your own chin bone and is fixed so it doesn’t move.  Over time the chin implant will feel like your own bone and like its your natural jaw.  You can also place it through your mouth but this usually has more associated issues with this approach.  First you have to put it past all of the bacteria that usually resides within your mouth and this can lead to increased risk of infection. Also through this approach you usually alter the way the chin muscle sits and this can cause some chin drooping because the muscle is not adequately attached to bone through this approach.  Also with through the mouth, the nerves that brings sensation to your chin and lower lips can be more affected and lead to more chances of numbness.

    The usual implant that is placed is silicone which is really compatible to your tissues.  Carbon is right above Silicone (Si) on the atomic chart.  Because they are very similar the body thinks of it like carbon and finds it less intrusive and doesn’t mount a large immune response to it.  Hence silicone implants are highly adaptable to the human body and there is very little risk of cancer, rejection, infection and other problems that you might get with goretex, med por, and other implants. Silicone implants that are solid have been placed in thousands of people with no problems over many years and decades!

    What are the options for sunken cheeks besides temporary dermal fillers?

    This is a questions I answered for someone that was wondering what option there were for sunken cheeks besides temporary Facial FillersFat Transfer or Facial Implants can give you the long term volume you desire in the cheek.

    Sunken cheeks is a natural aging process.  When you age you lose fat in your cheeks and this can lead to your tissues descending, wrinkles developing, folds developing, lower eyelid fat protruding, eyelid bags, etc.  Dermal fillers like restylane, juvederm, radiesse, perlane, are temporary and last from 6 months to a year or more.  Of these fillers radiesse is a bone product made up of calcium hydroxyapatite while the others are made up of hyuloronic acid.  There are more permanent fillers like silicone injections, and artefill.  The problem with permanent fillers is that they have the potential to cause infections, scarring and granulomas (balls of chronic infection) that can present years later after you think everything is okay.

    Other more permanent options for sunken cheeks include fat injections and silicone implants.  Silicone is very compatible to place in the cheeks.  The silicone atom is one step below carbon on the atomic chart and hence makes it really compatible.  The infection rate with silicone is very low and its use has a long history of safety.  Silicone implants can be placed within the cheek to give a person a more permanent volumizing and lift up sunken cheeks.

    Fat injections are another way of volumizing the cheek. Perrmanency depends on whether the fat survives.  This depends on many factors: technique, health of the patient, care of the patient after the procedure, etc.  If fat lasts for more than 6 months to a year, it will likely survive for many years sometimes 10-15 years or longer.
