What are the different types of facelifts out there, an explanation and interesting picture are shown.

Facelift (or face lift, quick lift, smas lift, short scar lift, facial rujevenation, thread lift, s lift, weekend lift, lunch time lift, one hour lift) is generally a procedure that lifts part of the face. It works to improve the jowl area (see figure 1),marionette lines (see figure 2), neck bands (see figure 3), neck laxity, double chin, and nasolabial folds (see figure 4). You can see the pictures showing where these anatomic structures are located. After a facelift, many of these things are improved as seen in the before and afters (see figure 5 & 6 ). There are many names for a facelift and you might have heard some of these names in the media. Essentially all facelifts including these advertised lifts that are labelled as minimally invasive all entail pulling on a layer of tissue called the SMAS or Superficial Muscular Aponeurotic System. The SMAS is the layer of tissue  that includes the facial muscles and platysma as seen in figure 7.  This layer of tissue is the layer that all facelifts lift or pull on to create the beneficial aspects of a facelift.  It does so because the SMAS is connected to other facial muscles and the pulling of the SMAS is transposed to the facial muscles that are located around the areas that need to be improved.  How do facelifts differ? They differ in terms of how much undermining the surgeon does in front of the ear, how deep the dissection goes, how long the incision is carried out and what other adjunctive procedures are included to improve the facelift results.  Most minimally invasive facelifts entail less undermining, they usually use sutures to pull up the SMAS and they limit their dissection and the extent of their incisions.  Minimally invasive facelifts that are advertised out there usually entail about 3-5cm of skin undermining in front of and behind the ear (see figure 8 blue line).  Usually they entail also sutures to pull up the SMAS after skin undermining wtihout elevating the SMAS. Most minimally invasive facelifts leave the SMAS intact and use sutures to fold the SMAS up and hence shorten the SMAS lifting the face structures.  The sutures typically grab the SMAS near the furthest part of the dissection and then are fixed and pulled to immobile tissue around the ear.  The incision typically runs above the sideburn and then between the ear and the face.  The incision can go in front or behind the tragus and then it is carried in front of the earlobe and then behind the ear normally running in the crease and then it goes along or within the hairline (see figure 8). More advanced facelifts extend their skin undermining (figure 8 Yellow line) and the length of the incision.  They are more likely to take the incision along the direction of the green dashed arrow than minimally invasive facelifts. They also are more aggressive in their treatment of the SMAS through elevation of the SMAS and further dissection underneath the SMAS. Even more complicated facelifts carry their SUB SMAS dissection (dissection under the SMAS) over the facial muscles and specifically the zygomaticus major and minor which are the smile muscles (see figure 7).  The reason for taking the dissection over the smiling muscles is to have more of an affect on the nasolabial folds.  This approach is very complicated and usually mastered by the most experienced facelift surgeons.  Here is a picture of this dissection over the zygomaticus major and minor (see figure 9 smile muscles are labeled and what you are visualizing is the zygomaticus major) which I did during an October 2009 facelift.  Traditional facelifts are also more aggressive in their treatment of the neck muscles under the chin which I will discuss in a different blog.  I hope this helps you in understanding the facelifts out there.

What is a silhouette lift and will it last?

This is a question I recently answered for someone asking about the silhouette lift which is a variation of a thread lift.

It really depends on how its done, is the bottom line.  If the sutures that they use are not properly placed and done without proper dissection the results will be temporary.  In order for the skin and muscle of the face to take on a new shape a significant amount of dissectiion is required to allow these tissues to readjust for the long term.  I’ve done over 1200 null so I know how the effects of a superficial lift is compared to a more traditional approach. There is a huge difference.  With the minimal dissection they purport, the results are going to be temporary.  Anything done in 45 minutes will not last years based on my pretty extensive experience with facelifting.

What are the options for reducing the size of your pores. Does a facelift work for this?

This is a question that I answered for a client who had questions regarding whether a facelift would work to reduce the pore size as well as acne scars:

Fat grafting, laser resurfacing, ipl are better options than a facelift (face lift/ lifestyle lift / quick lift / lunch time lift / mini lift / weekend lift / s lift / lifelift / thread lift ). Some newer studies and results from physicians more recently are showing that these modalities may be more beneficial to reduce pore size for people.  A facelift will not effectively do this but it can improve your wrinkles to an extent.  Fat grafting (or fat transfer / fat augmentation / facial fat transfer / fat injections) has been shown by some to regenerate the skin including reducing the size of your pores.  Stem cells in the transferred fat can rejuvenate the skin for years.  Laser resurfacing through the creation of new collagen can also reduce the size of the pores as well.  Ipl is another newer treatment for pore size.  Out of all the options fat grafting might be the best option for pore size and acne scarring.  More and more surgeons are using fat grafting as a way to improve acne scarring.

Turkey neck treatment in a person that is afraid of her history of keloids!

Keloids occur less in the facial area and certain ethnicities are more prone. It really depends on where you develop keloids.  Areas of high movement such as the knees, and shoulders are more likely to develop keloids.  The high part of the chest is another area that has a predilection for keloid production.  If you developed keloid in these prone areas the chances are that you develop keloids to the facial area are probably a lot less.  You are caucasian based on the pictures which leads me to believe that you are probably less likely to develop keloids in your face and neck area.  But you may be a rare case and more details about your keloid formation would help someone decide.  The options for improving your turkey neck include liposuction which will give you some mild to moderate improvement.  A formal neck lift (or necklift / platysmaplasty / lower face lift ) can entail only small incisions under your chin and behind your ear and hide very well and really improve your turkey neck (or submental fat, double chin / neck laxiy / neck bands / fatty neck).  Others have used lasers to tighten up the skin to a minimal degree.  But if you really want some results, I would do a more formal procedure and it can make a world of difference!

The short scar facelift is limited in the extent of the incision and amount of dissection

There are many different variations of a short scar face lift (mini lift / S lift / Weekend face lift / Lunch time face lift / Lunch Hour Face Lift / short incision face lift). This approach differs from traditional face lifting approaches by the limits in the length of the incision, the depth of incision, the amount of dissection, and extent of improvement.  It really depends on who you go to.  There are ways to limit the length of skin incision and still achieve amazing results through tissue distribution.  Many surgeons out there are making the short scar facelift more a “short cut” face lift where corners are cut and the results are essentially much less satisfactory.  However, there are some very knowledgeable surgeons out there that can use a short scar and do amazing things.  Short scar facelifts usually entail also superficial dissection to avoid exposing facial nerves and is deemed safer by many.

Non Surgical Face Lift

There are many questions out there regarding an alternative for a non surgical facelift.  There are ways to tighten up your face without making incisions.  One way to do this is to undergo a laser resurfacing. We have the lumenis Laser Resurfacing unit and this is proven to be the best one out there on the market. It is the safest and most versatile of all co2 laser units and this can improve your skin by decreasing the wrinkles, improving skin texture, decreasing pore size, tightening the skin and creating collagen under the skin surface to create some more youthful volume.  Co2 laser resurfacing can tighten your neck skin and laxity as well. One thing to remember is that the tightening from a non-surgical face lift with co2 resurfacing will be subtle as compared to a facelift but you can get some results from this.

The other options include using Botox , fillers, and fat injections that don’t require incisions.  Botox / Dysport is a neuromuscular agent that can make certain muscles weaker that helps decrease wrinkles that are from muscular contraction. When you age you tend to lose volume and this can be replaced with fillers that require injections as an option for a non surgical face lift. Facial Fillers are usually temporary, at least the safer ones and last from 6months to a year or more.  Restylane, Juvederm, Radiesse, Perlane are some options. These modes can be used for the wrinkles between the eyes called the Glabellar region, for the dark circles under the eyes, forLip Augmentation, and many other issues in the face.

Fat injection are another option for a non-surgical facelift and related to this is the YoungLift which I have refined.

Non surgical option for jowls and neck lift?

The neck and jowls are harder to treat without some type of surgery but there are options. There are many different Laser Hair Removal out there that are tightening skin without large incisions. These, I think, are still in the experimental phase but some really believe in this mode of non surgical null. The results are usually subtle.  And require more than one treatment for better results. A traditional neck lift is the best way to pull up the jowls and neck laxity.  Other options include fat injections in the face to volumize the face to pull the jowls and some of the neck laxity away.  Fat injections in the face can do wonders to make one look younger and are a great option to avoid long incisions that you are worried about.

Dark Circles, Jowls and whether a facelift of some sort can help

In terms of a null, you might be a little early to have one.  It would really be beneficial to see a picture of you without smiling. A facelift can lift your jowls and improve your nasolabial folds to some degree. it won’t help your dark circles, and bags under your eyes.  Most surgeons would advocate a lower eyelid blepharoplasty, but this could cause hollowing under your eyes and appear unnatural.  I would suggest filling your cheek and lower eyelid with some type of Facial Fillers.  With filler you could go with temporary fillers or try the YoungLift.  Fillers such as Restylane, Juvederm, Radiesse, Perlane, can all fill in the cheeks and partially the lower eyelid but are temporary and last 6months to a year or more.  Permanent fillers such as silicone and artefill are risky and I stay away from them.  Fat injections can help and if the fat survives for more than 6 months to a year it will stay there for years sometimes more than 10 years.

Approach to the midface and questions regarding the longevity of midface suture lifts and deep plane smas lifts

The suture suspension lift most likely refers to a midface lift and there are many ways to do this.  You can do it in a minimally invasive way through incisions in the scalp by the temple area.  All is needed are very tiny incision near your nasolabial folds to allow the sutures to pull up the middle part of the face.  Data on how long this lasts is not sufficient.  Most surgeons believe that effect is not as long lasting as other procedures.  A midface lift that is done traditionally can be done through incisions in the lower eyelid. This requires the middle part of the face to be released from the bone and then suspended to more superior structures.  This can help the nasolabial folds and elevate the midface cheek fat.  The results from this, in my opinion, are modest.  The reason for this is that the midface issue is really mostly a problem of volume.  Fat injections placed in the midface is a good option but not entirely perfect.  Implants can give the sufficient volume that is needed to replace what is lost in the midface area. But you have to feel comfortable with implants in your face.  The deep plane smas lift is very difficult to do and if you go to someone that doesn’t have a lot of experience with this it could be dangerous.    When done correctly, you can get a lot of release and pull on the nasolabial fold area and this can lift the midface in a more lateral and superior direction then just superior.  Most surgeons believe that the deep plane facelift will last longer than any suture only facelift 2-5 vs 8-10 year or more respectively for the suture lifts vs the deep plane.  These numbers are a pure estimation though but are somewhat supported by scientific review.

What to do about extra skin after losing a lot of weight

Anullis the best way to get rid of the extra skin in your face. I’ve done many facelifts on people who have gone through weight loss surgery and they work super on people who have extra skin from losing weight.  You should be able to get a super result.  Other options include the non ablative lasers and machines that tighten the skin.  Deep fx is another way of using a fractionated laser to help tighten the loose skin but these are subtle results. A facelift and neck lift are the best ways to improve this.  A facelift pulls up the muscle around your face and thenullgoes through an incision under your chin to tighten the neck muscles. Together they can really shape your neck after weight loss.
