This is what I wrote someone on Sept 28,2011:

 

Dr. Young,

Patient: I wanted to thank you again for your personable consultation the other day.  It’s never easy talking to a stranger about one’s insecurities, but you certainly made it comfortable enough that I am considering your recommendations.  Just a few questions I first need to ask.

Dr. Young: It was my pleasure.

Patient: Why did you choose the subcision with the chemical peel over other procedures? .

Dr. Young: I think that laser resurfacing may not fully cause those ice picks scars to come up to the level of the rest of your skin.  They (laser resurfacing / dermabrasion / Dermasanding) are better when the surfaces are more closely aligned. and leveled.  The chemical peel will get the ice picks to come up and the subcision will make them rise by stimulating from below.  Alternatively you can add fat grafting with the subcision for more of an effect. I would do the subcision / fat grafting first and then 1-2 weeks later do the chemical peels so that your skin is not too traumatized.

Patient: What are the alternatives?  And do they offer similar or less satisfactory results?

Dr. Young: You can cut around the ice picks and then raise them. This could help. They are more work though. You can add filler underneath them. You can excise a group of ice pick scars that are oriented in a pattern. You can do Dermabrasion but again resurfacing would be the last step. You can laser just the ice pick scars to get them to rise up (not as effective in my experience)

Patient: What are the side effects of these procedures?

Dr. Young: The side effects are mostly swelling, bruising.  Sometimes if you are too aggressive the scars could get wider but flatter. That is why I stage the subcision and chemical peels sometimes if the scars are deep and prominent and numerous.

Patient: How long is the procedure, and is it performed at the same clinic?  Do you schedule surgery on Fridays?

Eye bags at the age of 19 are they from my genes or lack of sleep by Dr. Philip Young of Aesthetic Facial Plastic Surgery: Eye bags are based on some genetics but most people have some element of it. I wouldn’t blame mother. Eye bags, Eye dark circles happen to almost everyone and it starts in the 20’s and 20’s. The cause is mostly the volume loss under your eyelid fat. The eyelid fat doesn’t get metabolized by the body and thus looks like it has grown. But it really is a loss of volume underneath it that accentuates it. Traditionally, people tended to remove the eyelid bags. This was not really treating the cause of what made them appear. The more natural way is to fill the volume loss. The options temporarily include fillers like restylane, perlane, juvederm. Long term options include fat injections, the young vitalizer, implants in this area.  Because you haven’t had anything done I would start with fillers before moving on to more longer lasting options. A lack of sleep can accentuate the condition. But mostly it is an issue of aging. The YoungVitalizer is another great option for lower eyelid bags and the one that I prefer.

Is there a permanent way of removing the vertical lines in between the eyes and eyebrows? by Dr. Philip Young of Aesthetic Facial Plastic Surgery: Brow lift with muscle excision and / or fat grafting / injections can be a solution for long lasting removal of vertical lines. First of all there is nothing that is permanent. Permanent, IMO, suggests infinity which is not possible.  But longer lasting results, instead of the results that you can achieve with botox toxin is possible.  There are ways to carry out browlifts where you can excise the muscle in between your eyebrows to decrease your ability to frown in this area. I have a live video demonstrating this if you are interested below. Fat injections all throughout the forehead is another option that can be done alone or in conjunction with a browlift and muscle excision. Another better alternative is the YoungVitalizer to treat these lines in between your eyes.

Residual neck bands and neck skin by Dr. Young of Bellevue: A Neck lift will help the banding your neck, but additional fat grafting / transer could also be needed in the future. This basic suggestion / opinion is from knowledge gained from doing nearly 1300 face and necklifts.  I don’t think a face lift revision will lead to a complete resolution to your issues unless you definitely approach the neck area with a neck lift. In my opinion, these are two different techniques (the face and neck lift).  The neck lift will markedly improve the neck bands and then after this you may still need to graft fat.  Based on the images, the neck probably is in need of some fat in some areas.  But I would wait to see how the neck lift improves your current status.  You may not need fat grafting at all. Only time will tell. You may also want to look at our new procedure called the YoungVitalizer.

Treatment of Malar Bags and the different options by Dr. Philip Young of Seattle | Bellevue: The Treatment for malar / cheek bags can be: 1. Direct excision of the malar bags; 2. Fat injections / transfer ; 3. Young Volumizer; 4. Lower eyelid Lift lower eyelid blepharoplasty. The advantages of the Direct Excision is that you can improve the area directly but there is the negative association of having an incision right on the bag area.  Fat injections / graftingare perhaps the most natural way to improve this area. The Young volumizer is like fat injections but done and developed by Dr. Young based on his theory on facial beauty. Lower eyelift can lift the extra skin and take away the fat under the lower eyelid. For your particular issue, I would recommend fat injections or the Young volumizer. This was a question that I answered for someone that had a lumps and collections in the temple area that was from a fat injection that was done by another surgeon. This is how I answered this question:

You can smooth these fat collections in the temple that happen after fat injections with a number of different techniques.  The options include steroid injections, mesotherapy, microliposuction, direct excision, more fat grafting to add around the lumps to camouflage the fat collections.  Mesotherapy is a way of dissolving the fat with the use of certain agents.  Steroids can be injected into the collection to dissolve it.  Microliposuction is the use of cannulas to accurately suction the collection away.  The last resort is to make an incision right above the collection and excising or taking the fat out directly. My technique that I use is called the YoungVitalizer which is a novel way of volumizing the face using your own tissue. Below is a picture of a temple augmentation with the YoungVitalizer

Deep scar from a spider bite with no improvement with 6 laser treatments by Dr. Philip Young | Aesthetic Facial Plastic Surgery

This was a question that I answered for someone that had a Deep scar from a spider bite with no improvement with 6 laser treatments . This is how I answered this question regarding Scar Revision for this Deep Scar:

Deep scars are hard to reverse with fillers, excision and fat injections can work.  This is in my experience.  Sometimes you are able to get some improvement with fillers. But what I’ve experienced is that the filler tends to be injected in the surrounding areas and not where you want them which is under the scar.  You really have to break up the scar tissue that has accumulated under the depression and this needs to be released.   Once release you can fill it with fillers or fat.  What I like to do is undermine the depressed area with different instruments and then fill it with fat.  This however will not improve any surface details / qualities like incisions or lines there.  The only way to improve that is to actually excise this type of scar.  I would really need to see a picture of the scar or the scar in person to better figure out a way to improve it.  Sometimes with the scar revision, you need to go under the skin to really release the collagen bands of scar tissue to alllow the skin to relax.  Then more fat grafting or some filling may still be needed.
